The pain may radiate down the arm or up into the neck. Posterior shoulder pain is often caused by overuse of the shoulder joint. It can also be caused by a fall or other injury to the shoulder. Treatment for posterior shoulder pain includes rest, ice, and pain medication. Physical therapy may also be recommended.
