Remembering chords is a very difficult thing to do, but once it becomes memorized it all seems so easy.
If you are trying to remember the notes in each chord, start by memorizing the major triads. There are only 12 keys in music, so if you memorize two chords per day, you can have all 12 major triads memorized within only 6 days!! From there you can begin to memorize the minor chords, diminished, augmented chords, and eventually all the different types of 7ths, 9ths, and so on.
As far as rhythms, if you practice reading rhythms EVERY SINGLE DAY, it will become much easier. You have to be willing to put the work in. The best advice I can give you is to break every measure apart, clap every rhythm out, and COUNT OUT LOUD while you clap the rhythms. Counting out loud is so important.
I hope this helps and let me know if there is anything else I can help you with.
